Understanding ABA CPT codes for billing and authorization

Category I codes 97151–97158 describe assessment, direct treatment, BCBA protocol modification, group services, and family guidance. H0032 covers NC Medicaid plan development. TargetFlo models each code on the authorization-to-claim roadmap.
What are ABA CPT codes?
CPT codes are the shared vocabulary between your center and payers. For applied behavior analysis, they document assessments (97151–97152), technician-delivered treatment (97153–97154), BCBA protocol modification (97155, 97158), and caregiver guidance (97156–97157). Most Category I ABA codes are billed in 15-minute units, but each payer sets rounding rules, daily caps, and telehealth policies.
Educational content only; not legal, clinical, or billing advice. Payer requirements change — confirm current policy with NC Medicaid, your MAC, or the payer before submission. AMA CPT updates and state Medicaid bulletins change frequently — verify current payer manuals.

Established codes for assessment, treatment, supervision, group, and family services. Typical rendering role shown — your payer may differ.

| 97153 | Adaptive behavior treatment by protocol | RBT / technician (supervised) | Timed codes: many payers use the 8-minute midpoint rule per 15-minute unit. |

97151 — Comprehensive assessment by a BCBA or qualified professional: direct observation, indirect analysis, scoring, report writing, and treatment plan formulation. Often billed alongside indirect report time; many payers cap daily units.
97152 — Assessment support performed by a technician under qualified professional supervision, commonly paired with 97151 on the same episode of care.
97153 — Direct one-to-one adaptive behavior treatment delivered by a technician under supervision after the initial assessment and plan are in place.
97154 — Technician-delivered group treatment targeting social communication and skill generalization with peers in a face-to-face group setting.
97155 — BCBA or qualified provider modifies the treatment protocol during a face-to-face session with the client, often directing a technician. Pure supervision without the client present is generally not billable as 97155.
97156 — Training for parents or caregivers to implement the treatment plan at home, with or without the client present.
97157 — Group sessions for multiple families or caregivers (typically without the client present) to share strategies and support implementation.
97158 — BCBA-led group adaptive behavior treatment for clients with similar goals — active treatment by the qualified provider, not technician-only group code 97154.
Medicaid
H0032 — service plan / BIP–POC development
Separate from direct treatment codes. In TargetFlo, the signed Plan of Care packet supports authorization submission.
H0032
Mental health service plan development (NC Medicaid)
Used in North Carolina Medicaid and similar programs for behavioral service plan / BIP–POC development. TargetFlo aligns Plan of Care drafting, clinician signature, and parent e-signature to the packet teams submit at authorization — separate from direct treatment codes 97153+.

Specialized & emerging codes
Temporary Category III codes (0362T, 0373T, 0770T) capture specialized or emerging services. Many commercial and Medicaid plans do not reimburse them — confirm coverage first.
| Code | Name | Summary |
|---|---|---|
| 0362T | Exposure behavioral follow-up assessment | Specialized follow-up assessment for destructive behavior, often requiring multiple technicians and on-site qualified supervision. |
| 0373T | Adaptive behavior treatment with protocol modification (multi-technician) | Two or more technicians deliver treatment for severe behaviors under on-site BCBA supervision; supervision cannot be billed separately during this service. |
| 0770T | Virtual reality adjunct | Category III code for VR used during adaptive behavior interventions; payer acceptance varies widely. |
Telehealth
Video, modifiers, and payer variance
Telehealth rules for 97155, 97156, and 97157 expanded in many markets after 2020, but coverage is not uniform.
- Video visits are the most commonly accepted telehealth modality for ABA CPT codes; audio-only is often excluded.
- Apply payer-specific telehealth modifiers and place-of-service codes — missing modifiers are a top denial reason.
- Medicaid programs revise telehealth rules independently; check your state bulletin before changing delivery mode.
- TargetFlo eligibility at intake captures payer context; payer rules store the telehealth modifier and place of service per payer.
Operations
Reduce denials before claims go out
Coding accuracy is necessary but not sufficient — authorization, documentation, and timing matter equally.
- Verify eligibility, prior authorization, and unit limits before the first session — most denials trace to missing or expired auth.
- For timed 15-minute codes, confirm the payer’s 8-minute midpoint rule before billing a unit.
- Apply telehealth and modifier rules per payer; audio-only is often not reimbursed for ABA CPT codes.
- Check concurrent billing and bundling edits (e.g., same-day combinations of assessment codes).
- Align session notes, treatment plan, and CPT selection so medical necessity supports each line.
- Track timely filing limits (commercial and Medicaid windows differ).
- Review state Medicaid bulletins and fee schedules when policies change mid-year.

Authorization management & claims
The same CPT table powers authorizations, scheduling checks, and claim lines.
In TargetFlo Billing & Claims, each authorization stores approved units per CPT code. Sessions and signed notes link to that authorization so claim math is units × contracted rate, not a spreadsheet reconstructed at month-end. Eligibility (270/271) feeds the first link in that chain.
