Why ABA Charges Get Held: 14 Reasons and How to Fix Each One

Illustration for Why ABA Charges Get Held: 14 Reasons and How to Fix Each One

The 14 reasons an ABA charge is held before it reaches a claim — no insurance, no authorization, missing NPI, rate, diagnosis or place of service, too many modifiers, timely filing — and how to fix or release each.

When a signed session can't become a correct claim line, TargetFlo doesn't guess — it holds the charge with a reason. Billing → Charges lists every held charge; fix the data and click Recheck, or release it with a note where that's allowed.

# Hold reason How to fix it
1 No primary insurance on the date of service Add the client's primary insurance with the right effective dates
2 No payer ID Add the payer's electronic ID to the insurance company
3 No payer rule (missing or inactive) Set up the payer in Payer rules
4 No authorization Record the authorization line for the code and dates
5 Authorization used up Request more units or start the reauthorization
6 Rendering NPI missing Add the renderer's NPI (when the payer requires it)
7 Missing rate Add the contracted rate for the code in Payer rules
8 Zero units Check the session's start and end times
9 Past timely filing Release with a note if you will still submit, or write it off
10 Missing diagnosis Add a primary ICD-10 diagnosis active on the date of service
11 Missing place of service Set the payer's place-of-service code for the location type
12 More than four modifiers Review credential, authorization and telehealth modifiers
13 Session not completed on the calendar Complete the calendar event (its authorization check may have blocked it)
14 Session voided Nothing to bill; a sent claim gets a correction

What fills a charge automatically

Date of service from the calendar; units from actual minutes and the payer's rounding; the authorization line the calendar booked; credential modifiers (HM, HN, HO), authorization modifiers and the telehealth modifier; place of service; the payer's code mapping and rate; and the supervising provider.
